Providing Medical Care for Medically Ill Elders in the Community

CenterLight Healthcare (formerly Comprehensive Care Management) is a managed care organization which provides interdisciplinary long-term health care services for chronically ill and disabled residents of  New York and Long Island.

We will meet with Shari Mann, a graduate of our program, and other CenterLight staff, to hear about a remarkable program (the PACE program) that helps maintain the sick elderly in their homes and community.

 Some questions I would like you to think about and ask the staff at CenterLight:

  1. How can we maintain our infirm elderly in the community? 
  2. What are appropriate goals of care? 
  3. What issues must we face in trying to provide this care?
  4. In a culturally diverse place like the Bronx, what steps have been made to develop a culturally sensitive program?  Have there been particular challenges?
  5. Which of our patients would be good candidates for CenterLight? 
  6. How can we integrate palliative and end-of-life care into a general medicine program for the infirm elderly?
