March 21: Home Health Care: Montefiore Home Health Care Agency (NO FACULTY)

Take own transportation (if car: park in garage on Washington St ,vouchers provided). No faculty who will accompany you on this trip.

8:30 -12:30     Montefiore Home Health Care, 1 Fordham Plaza, 11th Floor,  Conf  Rm A,  Bronx NY 10458

Residents should come to 1 Fordham Plaza and plan to arrive at 8:30am. You must go to the 11th Floor to get buzzed in and then will head up to the 12th floor conference room. They will serve a light breakfast.

  • Please make sure to have completed the pre-visit questions by Wednesday, March 14th and email to Angela Schonberg (email address above). Post-visit questions should be completed and emailed to Angela by March 28th.

On Wednesday morning, March 21st, you will be meeting with the Montefiore Home Care staff, and going out with them as they make home visits.

No faculty will accompany you on this trip.

Please get to Montefiore Home Care by 8:30 am on your own -  It’s located at 1 Fordham Plaza, Bronx NY 10458

If you drive, park in garage on Washington St , and Monte Home Care will provide parking vouchers (directions below).

Some questions I would like you to think about and ask the staff (and patients/families):

  1. What critical clinical data can you gather on a home visit that is not available to you in the office, clinic, or hospital?
  2. What impact do home visits have on: patients, families, and physicians?
  3. Are there particular patients/situations where a home visit or house call would be particularly effective?
  4. What type of home care services do insurance companies cover?  Is this appropriate?
  5. Are there certain chronic disease where home care services would be particularly valuable?
  6. Which of your patients could utilize these services?  How would you integrate the information provided by the home care team in your patient care?

The nearest form of public transportation is the MetroNorth Railway - Fordham stop.

Otherwise, by Subway - you can take the #4 train to the Fordham stop. From there, take the BX12 bus east on Fordham until Fordham Plaza. Or else, it's about a 20 minute walk east on Fordham.

Directions by car from Monte: Go west down Gun Hill Rd,

            Take a right at the bottom of the hill, onto 3rd Ave

            Take a left onto Fordham Rd,

            Pass 1 Fordham Plaza on your left, and then take first right onto Washington Ave

            Go 1 block and take first left onto 158th St

            Enter garage on left, approx ½ block down 15th St
