Transitions Clinic

Transitions Clinic is a collaboration between PCSM, CHCC and The Osborne Association, which provides services and programs for recently incarcerated people and their families.  The goal of Transitions Clinic is to provide individuals with chronic diseases who have been recently released from correctional facilities access to timely and respectful health care, including medical services and addiction treatment.  The goal is also to give residents experience in caring for individuals who are reentering the community after release from city jail or state prison, and in addressing both their medical and psychosocial needs.  Transitions Clinic takes place on Friday and Saturday mornings at CHCC.  The resident assigned to Transitions Clinic will typically see three to four recently incarcerated patients and will precept with Montefiore's Dr. Aaron Fox or occasionally with Dr. Jonathan Giftos, the Director of Clinical Education for Correctional Health Services at Rikers Island (and former PCSM Chief Resident).
